To be recognized as a Patient Centered Medical Home, practices submit data and reports to NCQA (National Committee for Assurance) to achieve recognition. Recognition is achieved by submitted reports and data based on the following standards:

* Patient Centered Access
* Team Based Care
* Population Health Management
* Care Management & Support
* Care Coordination & Care Transitions
* Performance Measurement & Quality Improvement

These standards show the quality of care we provide to our patients as well as validate our patient outcomes. The submissions process takes about 5 months to gather the data and run reports for each of the six standards. Once submitted, NCQA uses a scoring system for each standard, with the highest score to achieve being 100. Associates in Primary Care Medicine is proud to announce we received PCMH Level 3 recognition from NCQA with a perfect score of 100!
